As a Healthcare Consultant, you’ll play a crucial role in ensuring members receive the best possible care and support. You will be responsible for evaluating member needs, developing care plans, and coordinating services to address those needs. This involves collaborating with internal and external partners, including case managers, medical directors, and healthcare providers, to overcome barriers and achieve optimal outcomes. Your ability to connect with people, understand their unique situations, and advocate for their well-being will be essential to your success.
What You’ll Do:
- Conduct comprehensive evaluations of member needs and eligibility through the use of care management tools and data analysis.
- Develop and implement individualized care plans, monitoring progress and making adjustments as needed.
- Collaborate with case managers, medical directors, and other healthcare professionals to ensure coordinated and effective care.
- Identify and address high-risk factors and service needs that may impact member outcomes.
- Provide coaching, information, and support to empower members to make informed healthcare decisions.
- Utilize negotiation and motivational interviewing skills to engage members and promote positive lifestyle changes.
- Maintain accurate and timely documentation of care in accordance with regulatory guidelines and company policies.
Must-Have Qualifications:
- Bachelor’s degree in behavioral health or human services (psychology, social work, marriage and family therapy, nursing, counseling, etc.) OR a non-licensed master-level clinician with a degree in one of the above fields.
- 2+ years of experience in behavioral health, social services, or a related field.
- Proficiency with standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint.
- Excellent analytical, problem-solving, communication, organizational, and interpersonal skills.
- Ability to work independently and effectively manage multiple priorities.
Nice-to-Have Qualifications:
- Case management and discharge planning experience.
- Managed Care experience.
- Experience working remotely and collaborating with virtual teams.
The target hiring compensation range for this role is $25 to $28 an hour. Compensation is based on several factors including, but not limited to education, relevant work experience, relevant certifications, and location.
